You have the right to be informed about the services available to you. | |
You have the right to be assessed to receive services without judgement or discrimination | |
You have the right to provide critical feedback or make a complaint about the services provided. | |
You have the right to be treated with dignity and respect. | |
You have the right to be recognised and treated as an individual | |
You have the right to privacy and confidentiality. | |
You have the right to choose alternative service providers for supports. |
Your responsibilities
You have a responsibility to act in a way that respects the rights and safety of your Support Worker. HELLO LIFECARE SERVICES may withdraw supports should a worker’s health and safety risk is identified. | |
You have responsibility for the outcome of any decision you make. | |
You have a responsibility to follow through with any tasks that you have agreed to do in your goal plan. | |
You have a responsibility to meet with your support worker or inform HELLO LIFECARE SERVICES if you won’t be able to keep the appointment. | |
You have a responsibility to inform HELLO LIFECARE SERVICES or your NDIA of any complaints or concerns that you have. |
